Please include the following on all written excuses:

• Date excuse note was written

• Student's FIRST and LAST NAME - some students have last names that are different from their parent's/guardian's last name.  We can only search for a student by last name in PowerSchool. 

• Date(s) of student's absence(s)

• Specific reason for student's absence(s) - See pages 4 -5 (D. Excused Absences of the MPS Student Conduct Manual) for a list of acceptable reasons for student absences

• Printed parent's first and last name

• Parent/ Guardian Signature
