First Class A Offense - Teacher documents incident on the form to start tracking and makes contact with the parent.

Second Class A Offense - Teacher documents incident on the form, contacts parent and tries different consequences.

Third Class A Offense - Teacher documents incident on the form, parent conference, and alert administrator on the third offense

Fourth Class A Offense - Office Referral
